What is Seniors•At•Home? Seniors•At•Home, a program of Jewish Family and Children's Services, is the Bay Area's most comprehensive provider of compassionate, quality care to older adults. It is a wide range of services that help older adults live safe, independent lives in their own homes. Seniors•At•Home serves San Francisco, Marin, and the Peninsula. What is home care? Compassionate caregivers assist individuals of all ages recuperating at home after illness or hospitalization and help older adults manage daily life. Home care services are flexible and responsive to your needs. RN's are available to visit you for a personalized health consultation and to coordinate your health care needs with your physician, family and caregivers. Seniors•At•Home is a California state licensed Home Health Agency. Who can benefit from home care? Home care is useful to individuals who feel they need some assistance with activities of daily living. For example, an individual who has difficulty shopping and preparing his or her own meals or bathing independently may benefit from the services of a caregiver who can assist with these tasks. Family members, friends, and others may also benefit from home care because it can provide respite from caregiving as well as the reassurance that their loved one's needs are being met even when they cannot provide the care themselves. How does Seniors•At•Home find the right caregiver for me? Based on an in-home assessment of your individualized needs and preferences, Seniors•At•Home home care takes responsibility for identifying the best available caregiver and matching his or her skills and characteristics with your preferences. The Seniors•At•Home team will work closely with you to ensure that the match is a good one, and that all home care services are well coordinated and meet the highest professional standards. An RN and Staffing Coordinator are available 24 hours/ 7 days a week to address your needs. All Seniors•At•Home caregivers are employees of Seniors•At•Home. They are bonded, insured, and background screening is performed. All employer responsibilities are handled, including taxes, insurance, benefits, and workers' compensation. Our caregivers are fully trained and supervised. How much does home care cost? The cost of hiring a caregiver will depend upon the types of services provided. Home care services can be provided on an hourly (4 to 24 hours a day) or a live-in basis. Depending upon the individual's needs, home care is often the least expensive approach. Some Long Term Care insurance policies cover home care services. Seniors•At•Home's fees are competitive with other local home care agencies. For specific details, please contact us at (415) 449-3777. Does Long Term Care insurance pay for home care? Yes, many Long Term Care insurance policies include home care if certain requirements are met. What is Care Management? At your convenience, professional staff comes to your house to review those aspects of your home and lifestyle that impact your ability to live a safe, healthy, independent life. It includes a thorough in-home assessment, a personalized plan of care, all of the resources and steps to implement that plan, and ongoing monitoring and follow-up. Does my managed care plan pay for Care Management? Although most managed care plans do not cover social services, Seniors•At•Home has been awarded contracts with United Health/Health Net Senior Plus, and Secure Horizons for our award winning care management services. Following an in-home assessment, Seniors•At•Home will submit a statement of need. Upon review of your specific situation, it is possible these managed care plans will approve care. If you belong to another plan, please contact your managed health care provider directly. Will Seniors•At•Home provide me with financial assistance to pay for its services? Some clients may be eligible for a sliding scale or a scholarship for short-term assistance. Eligibility is determined on a case-by-case basis. |
