Navigating the healthcare system is difficult, especially for people who have been diagnosed with serious or life-threatening conditions. It is hard to absorb medical information and consider all of the decisions that need to be made.

Seniors•At•Home can support you at this challenging time by opening the lines of communication, facilitating collaborative care between you and your healthcare team, and providing information and support to you and your family. Together, we will consider your complete needs: mind, body, and spirit.
