Community Partnerships and Programs

Seniors•At•Home works with a wide range of professional and community organizations, service providers, and funding agencies to ensure that our clients receive the best service possible.

Partnerships

  • San Francisco Village – A community-based membership organization that empowers adults to live in their own homes as they age, by offering a network of high-quality resources, services, programs, and activities. Seniors•At•Home is a preferred provider.
  • Avenidas Village – A membership program offered through the non-profit agency Avenidas, designed to help people remain in their homes as they age by providing personalized assistance, services, and amenities. Seniors•At•Home is a preferred provider.
  • Marin Village – Marin Village assists and enriches the lives of independent elders who want to continue to live in their own homes in Marin County, by providing referrals to health and home maintenance services and information about social and cultural programs. Seniors•At•Home is a preferred provider.
  • Kaiser Permanente Hospitals – Kaiser delivers healthcare services throughout the Bay Area. Seniors•At•Home is contracted with Kaiser to provide conservatorships for their most at-risk clients.
  • Mills Peninsula Medical Group – A physician-owned and operated, multi-specialty Independent Practice Association. Seniors•At•Home is partnered with MPMG to provide care management for their frailest clients.
  • Family Caregiver Alliance – A nonprofit organization that addresses the needs of family and friends providing long-term care at home. Seniors•At•Home is partnered with the FCA to provide respite home care services for their clients.
  • Veterans Administration – Provides patient care and federal benefits to veterans and their dependents. Seniors•At•Home is partnered with the VA to provide home care to veterans as well as respite care for veterans'  family members.
  • Bay Area Vital Link – Provides seniors and people with disabilities with medical-alarm services that enhance their safety, independence, and peace of mind. Seniors•At•Home is partnered with Vital Link to provide this service at no cost to Holocaust survivors.
  • Pathways - A community-based, nonprofit organization that provides health services at home to families in the San Francisco Bay Area, including home health care, hospice, community education, and grief support. Seniors•At•Home is partnered with Pathways to provide respite home care services for their hospice clients.
  • Sutter Foundation – Sutter Health is a network of doctors and hospitals in Northern California. Seniors•At•Home is partnered with Sutter Foundation to provide respite home care services for their clients.
  • Multipurpose Senior Services Program - MSSP is a state-wide, voluntary geriatric care management program that enables frail seniors who otherwise would have difficulty living independently to remain in their own homes. In Marin County, MSSP is locally administered by Seniors•At•Home.
